Forms for the President and Vice President
Club Evaluation Use this form to evaluate your club and compare it to an ideal one before you establish goals for the year.
  
Club Structure Use this organization chart to assign members to committee roles.
 
S.M.A.R.T. Goals Every club should have well-designed written goals. This form helps the club to create goals that can be accomplished.
Forms for the Secretary
Roll sheet The secretary needs to maintain records of the number of people at each meeting and event. While this form is specifically for meetings, it can also be used for activities.
Circle K Monthly Report Form (MRF) Circle K secretaries use this form to submit monthly reports (MRFs). This form is a Microsoft Word document.
 
Key Club Monthly Report Form (MRF) Key Club secretaries use a different monthly report form that required Microsoft Excel to complete. This form automatically calculates the information necessary for the annual report.
Forms for the Treasurer
Emergency Card All members should complete this form. It insures that your Kiwanis advisor can seek medical attention if the members is injured at a service or social activity. The treasurer should collect this form as part of the process of bringing a new member into the club. The form, when completed, must be given to the Kiwanis advisor.
 
Photo Release In order for your club or Kiwanis to put a photograph of a student on a brochure or website, we must have permission of the student (if he or she is 18 year or older) or his or her parent. The treasurer also collects this form as part of the membership process. The form, when completed, must be given to the Kiwanis advisor.
 
Deposit Slip When the treasurer collects money and deposits it with Kiwanis, he or she must complete this form to insure that the money is credited to the correct account.
 
Transaction Register Use this form to keep track of all income and expenses. The treasurer will be given two transaction register booklets -- one for the club's school account and one for its Kiwanis account. This form is only needed when the register booklet has been filled and extra pages are needed.
 
Treasurer's Monthly Report
 The treasurer completes this form each month to enable him or her to track funds to which the club has access. In addition to keeping a copy of the form in the club files, the treasurer should provide one to your Kiwanis advisor and one to your school advisor.
Forms for Committee Chairs
Committee Meeting Report Each committee should complete notes like this form to report to the Board of Directors and insure that all committee members know what their responsibilites are.
 
Event Sign-in Sheet  Keep track of the people who participate in your event. This will enable your president to make recommendations about awards at year-end. In addition, it will serve as a record for students who need service-hour credit for school awards or graduation.
 
Membership Application  Use this form to gather information about prospective members.
 
Project Planning Guide This form is a guide for project planning. it works equally well for service projects, fund raisers, and social events.
 
Project Checklist  Use this form to make sure that you have not skipped any steps in planning a successful event of any kind,
 
Project Evaluation When you have completed any activity, you should review it to make sure that the next event will be even better. Use this form to complete the evaluation; then submit it to the Board of Directors as part of your final project report.
 

Project Roll Sheet You may prefer this form to the Event Sign-in Sheet to track participation, Both forms serve the same purpose. Use the one you like best. 
Forms for Personal Interest
Scholarship.pdf Use this form to keep track of activities that will enhance your Key Club or Circle K scholarship application. Start using this form as soon as you join your club, even if you will not be eligible for a scholarship for several years.
